Application form to amend/submit previous financial year data
In limited, specified circumstances, a provider may need to submit new session reports late, or vary or withdraw session reports after the date on which the department restricts access to the child care subsidy system. This form is the channel by which providers apply to the department to make changes in a previous FY to ensure that there is no financial impact on child care providers and families.
