This checklist will help case managers, health professionals, educators, community organisations and other relevant support services provide evidence to support an ACCS child wellbeing application.

Include:
- organisation name
- staff member name
- position/title
- contact details
- date completed
- organisation letterhead
- length of involvement (if available)
- signature (written or electronic)
- confirmation that consent requirements have been met (where required).
About the evidence
Confirm that the information provided:
- is based on your direct knowledge, observations or professional involvement
- is current and relevant to the child's circumstances
- is factual and evidence-based
- does not include assumptions, opinions or speculation.
Include:
- child’s name
- impact on child wellbeing
- description of family circumstances
- services currently being provided
- relevant dates or timeframe.
The evidence should clearly explain:
- why the child needs additional wellbeing support
- how the child's circumstances align with the eligibility criteria
- how the circumstances are affecting, or may affect, the child's safety or their physical or mental wellbeing.
Where relevant, include details of:
- safety concerns
- housing instability
- family violence
- parental alcohol or drug misuse
- serious illness, mental health concerns or hospitalisation affecting a parent or carer
- court orders
- other factors contributing to the child's vulnerability or risk of harm.
Before you submit
Confirm that:
- the information has been reviewed for accuracy
- an appropriate contact person is available or follow-up (if required)
- all relevant supporting evidence has been included
- all supporting letters have been signed and dated.
Remember
Good quality evidence is factual, current and clearly identifies the child's circumstances. It explains why the child needs wellbeing support or is experiencing, or at risk of experiencing, harm.
